We are seeking recruitment of a short term contract within the Community Assessment and Rehabilitation Team (CART) – ( Adults & Older People ). This post will bebased at the Memorial Hospital (Shooters’ Hill – SE18).


Social Worker Level 2                                 

Short Term Contract –Full Time 35hrs           

ELTHAM CART                                            

A short term contract (up to 4 months) has arisen for a senior social work position in one of the Community Assessment and Rehabilitation Teams: Social Worker L2 (Eltham CART). 

You will be working closely alongside health professionals such as Occupational Therapists and Physiotherapists – the CART teams provide an integrated service with a focus on Rehabilitation, in order to enable Service Users to achieve best outcomes, improved wellbeing and maintain and maximise their independence. The Teams undertake assessments and reviews of both new and existing service users.  

You must be a professionally qualified and registered social worker with recent and substantial post-qualifying experience of work at the interface between social care and health.

You should have a sound knowledge of the Personalisation agenda and understand the legislative framework which underpins social work practice today.  You must also have knowledge and experience in the management of Safeguarding Adults Enquiries and be able to carry out Mental Capacity Assessments.
